Introduction

Thank you for your interest in referring a participant for behaviour support services.

At Active Positive Behaviour Support we provide specialist behaviour support in line with the National Disability Insurance Scheme (NDIS) guidelines. Our goal is to create tailored, person-centred behaviour support plans that promote positive behaviour, improve quality of life, and reduce behaviours of concern.

This referral form is designed to help us gather essential information about the participant's needs, preferences, and goals.
